Home and Community Care Support
Providing personalised support to help you live safely at home and participate in your community.
Gateways is funded under the Home and Community Care Program (HACC-PYP) to provide one-to-one and group support for eligible clients who are not currently receiving or eligible for National Disability Insurance Scheme funding. Our service focuses on improving and maintaining independence, building confidence, and supporting participation in community activities.
Eligible clients include people under 65 years of age who experience difficulties performing daily living tasks and their carers, as well as Aboriginal and Torres Strait Islander people under 50 years of age. Our experienced team works closely with each individual to provide practical, person-centred support tailored to their goals and needs.
